SurveyMonkey

An online survey creation tool. With a free account, up to 10 questions can be made and 100 responses collected. There is a wide range of question types (dropdown, multiple choice, comment box etc.).

Evaluation: If you are looking to make a short questionnaire, SurveyMonkey may be a practical tool for it. Teachers can also have students create their own surveys. Average to advanced IT skills needed.
